How it works: the six step path to better health

Step 1 - Applying

Your path to better health begins with us getting to know you and your needs. We encourage you to begin your journey by contacting us. Give us a call at 1.866.642.9466. We would love to hear from you.

To take the first step in the process, you need to complete an application. This will become the foundation of our intake and care process. Call us to obtain your application form. You can then complete and send it to either our U.S. or Canadian office.

Once we receive your application, our Health Care team will assign a Personal Care Manager who will call to discuss your needs and your best options. If it appears that treatment through Axiom Health is right for you, your Personal Care Manager will collect additional medical information and answer your questions on fees and payment options.

Consulting Surgeon for your care

Your Personal Care Manager will then arrange for you to be interviewed by our Consulting Surgeon, who may wish to confer directly with your family doctor. This is important to determine you get the best and most appropriate care for you.
